Prescribing Support

We can offer dedicated medication review clinics or work with you to identify how to integrate these reviews into our bespoke service. We can undertake clinical medication reviews with patients with multi‐morbidity and polypharmacy and implement prescribing changes, including arranging and following up on relevant monitoring tests.

Being the first point of contact for any medication or treatment related queries will maximise clinician availability for medical consultations. We can provide dedicated time for patient-facing/video or telephone appointments for questions, queries and concerns about medicines. This can often provide an opportunity to intervene on areas for improved care.

With a strong background in multidisciplinary care, we can identify and liaise with any specialist healthcare staff or services. This ensures high quality, evidence-based outcomes for any medicines queries. We answer relevant medicine‐related enquiries from GPs, practice staff or other healthcare teams. Our pharmacist will suggest and recommend solutions where possible and provide follow up for patients to monitor the effect of any changes.

Reconciling medicines following registration of new patients, discharge from hospitals, or moving into intermediate care or care homes improves patient care and reduces medicines-related errors. We can identify and manage medicines to be prescribed or any changes required. Working with patients and community pharmacists, our services ensure patients receive the medicines they need. This process involves following up any ongoing treatment needs including dose titration and booking of follow up tests.

We have experience in managing the repeat prescribing reauthorisation process and can review medicines in line with practice protocols for repeat medicines. If required, we can undertake a portion of the repeat prescribing workload.

We are experienced in managing acute prescription requests, prescribing where appropriate. Within this service we can triage or recommend treatment for patients with common/minor/self-limiting ailments. Where necessary, patients are referred to another healthcare professional for the appropriate level of care within an appropriate period of time. Clinical Services

Some examples of clinical support services are provided below, this is not a comprehensive list as pharmacist specialist training varies widely. Contact us directly for specialist advice on your requirements.

We can provide various levels of care for patients taking rheumatoid medications requiring monitoring. Where practices have a robust protocol in place we undertake prescribing of medications. We can also support development of practice procedures to move monitoring and prescribing of DMARDs from the medical workload to pharmacist caseload.

From the point of diagnosis, we can initiate, titrate and monitor pharmacological treatment in line with national guidelines and individualised patient care. Undertaking annual reviews, we have an opportunity to support health improvement and change treatment guided by patient experience and up to date evidence.

As clinically trained practitioners we can undertake blood pressure monitoring as indicated and initiate changes at the point of care.

As a long term condition, diabetes is a growing epidemic within our society and recent times have provided an extra barrier to long term care.

Targeted management of diabetes and associated complications is a public health priority.

We can support monitoring of bloods, medication review, lifestyle advice, clinical assessment, cardiovascular risk assessment and referral to support services in line with local procedure and national guidelines.

Poor asthma control has been identified as an area of priority within the National Review of Asthma Deaths. As experts in medicines and inhaled therapy treatments, our pharmacists can undertake annual reviews or treatment assessments of patients living with asthma and recommend changes to improve control.

We have the skills required to undertake practice based spirometry testing for assessment by medical practitioners. In line with the GOLD guidelines we can assess patients living with COPD and advise or step up treatment as necessary.

Audit and Services Development

We have knowledge and experience of recommending changes to reduce the prescribing of these medicines to high‐risk patient groups.

 

We can implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance to preserve practice time for patients.

We offer pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways, patient information leaflets or standardising patient care).

Upon request, we can monitor practice prescribing against local formulary, SMC approved medications, unlicensed/high cost medicines and medicines that should be prescribed by hospital doctors or subject to shared care.

We are able to audit your practice’s compliance against NICE technology assessment guidance or other guidance as requested.

As part of our core value of improving patient care, we can review cohorts of patients at high risk of harm from medicines through computer searches or at the direction of your individual practice.

We analyse, interpret and present medicines data to highlight issues and risks to support decision making where necessary. Where invited, we can present data at a practice level and re-audit within an agreed timeframe to demonstrate change.

To support revalidation, we can undertake clinical audits of prescribing as directed by GPs. We can feedback the results and implement changes in conjunction with the practice team or on an individual level.

 

We can arrange and provide education and training to your healthcare team on therapeutics and medicines optimisation from field experts.

In addition to common long term conditions training, we offer specialist training in learning difficulties and mental health medication management, with opportunities to address individual patient concerns and areas for practice improvement.
